What I Said

I argued that Boston Scientific was still a constructive Buy because growth, profitability, and cash flow were all trending in the right direction. I pointed to Q1 2026 revenue growth of 11.6%, EBITDA growth of 32.3%, net income growth of 99.0%, and TTM free cash flow of $2,807.1M as evidence that the operating profile could support the valuation. I also flagged reimbursement pressure and acquisition-led execution as the main swing factors, with margin expansion and integration as the key tests.

Peer Comparison

BSX (this call)-7.4%
ABT+16.6%
EW-0.5%
MDT+18.4%
SYK-8.1%
ZBH+4.4%

Why It’s Working (Or Not)

A long position in BSX has lost 7.4% over this tracking window, so the Buy call has not worked to date. That outcome leaves my original valuation-and-execution thesis unconfirmed: the stock has moved lower even though I expected the growth and cash flow profile to keep the multiple defensible. The peer set has been mixed, with ABT, MDT, and ZBH higher while EW and SYK have been roughly flat to lower, so BSX has not simply followed a uniformly weak group. On balance, the specific case I made for durable operating momentum has not translated into positive price performance.

What Could Still Prove Me Wrong

If BSX can recover and hold above my entry area while the market continues to reward the same growth-and-cash-flow profile I highlighted, that would help repair the call. More importantly, I would need to see the reimbursement pressure and acquisition integration risks show up in the numbers in a way that clearly undermines margin expansion or cash flow durability. If the stock remains weak while peers continue to stabilize or advance, that would further argue my original valuation support was too optimistic.
